We are committed to protecting your privacy and maintaining confidentiality regarding your personal and recovery-related information.

2. Information We Collect

We may collect the following types of information:

  • Personal Identification Information: Name, address, phone number, email, date of birth, and emergency contact details.

  • Health-Related Information: Surgical procedure type, recovery details, medication lists, allergies, discharge instructions, and other information voluntarily provided on your intake questionnaire or during consultation.

  • Payment Information: Billing address and payment method (processed securely through a third-party payment provider).

  • Communication Records: Emails, text messages, or notes related to scheduling, care coordination, or customer service.

3. How We Use Your Information

Your information is collected and used solely for legitimate business and service-related purposes, including:

  • Evaluating suitability for non-medical companion caregiving services.

  • Coordinating your care schedule and recovery logistics.

  • Communicating with you, your designated emergency contact, or your healthcare provider as necessary for your safety.

  • Processing payments, invoices, and receipts.

  • Maintaining business and legal records as required by state and federal law.

4. Confidentiality and Disclosure

We treat all client information as private and confidential. We do not sell, rent, or share your personal data with third parties for marketing purposes. Your information may be shared only in the following limited circumstances:

  • With your written consent.

  • To comply with legal obligations (e.g., court order, subpoena).

  • To protect your health or safety in an emergency situation (e.g., calling 911 or notifying your emergency contact).

  • With third-party vendors who assist in billing, scheduling, or recordkeeping, provided they agree to maintain confidentiality.

We use reasonable administrative, physical, and electronic safeguards to protect your information.
All client intake forms, service agreements, and records are securely stored in password-protected digital files accessible only by authorized personnel of Safe Haven Recovery Care LLC.

While we are not a medical facility and therefore not a “covered entity” under HIPAA, we voluntarily uphold similar privacy and confidentiality standards to protect your sensitive information.

6. Retention of Records

Client records, consents, and related documentation are retained for a minimum of seven (7) years following the conclusion of services, unless a longer retention period is required by law.

7. Your Rights

You have the right to:

  • Request access to your personal information on file.

  • Request correction of inaccurate or incomplete information.

  • Request deletion of your information when legally and contractually permissible.

  • Withdraw consent for communications at any time by contacting Safe Haven Recovery Care in writing.
